The following states have historically used their own forms due to state regulations or laws. These states include:

Florida
Georgia
Hawaii
Louisiana
Maryland
Mississippi
Nevada
North Carolina
Oklahoma
South Carolina
Texas

Note that some of these states are evaluating whether to withdraw their old state forms and use the NPMA-33 but none of the above states have taken final action at this time.
